I think based on everything it's highly implied but not outright said.

I suppose she could have been severely disabled by Ogata's assault which resulted in the death of her baby? Maybe that's why Tanaka says she's in a safe place.

I read it over and they still didn't explicitly say she was dead. He can avenge his wife for her pain caused, even if she's not dead. I'm not saying she is alive, however. I just don't accept implications in manga/anime.

I took a quick look and I didn't see any line saying she was dead. If it's there, I must've missed it.

Yeah, Hayato says she was killed on page 10.
Also, you could tell she's dead since Ogata asked him about avenging his master, wife, and child.
